Privacy & Email policy

We are committed to protecting the privacy of patient information and to handling your personal information in a responsible manner in accordance with the Privacy Act 1988, the Privacy Amendment (Enhancing Privacy Protection) Act 2012, the Australian Privacy Principles and relevant State and Territory privacy legislation (referred to as privacy legislation).

The Privacy Policy explains how we collect, use and disclose your personal information, how you may access that information and how you may seek the correction of any information. It also explains how you may make a complaint about a breach of privacy legislation.

This Privacy Policy is current from 1 March 2017. From time to time we may make changes to our policy, processes and systems in relation to how we handle your personal information. We will update this Privacy Policy to reflect any changes. Those changes will be available on our website and in the practice.

Patients may contact their doctor by email.

admin@coomerafamilypractice.com.au, although this is not the preferred method of contact. Please do not include any personal or confidential information in the email as it is not a secure method of communication

Never use email for urgent matters.

Your doctor or designated staff member will respond to your enquiry by telephone as we are unable to guarantee confidentiality through email.

As part of our commitment to providing you with the best service and health care, it is important that we collect up to date and relevant information from you and store it in your patient record file.

You can, at any time, request in writing, a copy of your patient record.

The purpose of information collection is so that the doctor can safely and timely provide medical care and attention that can be tailored to your specific needs and requirements.

At certain times, we may have to forward particular parts of your information to other health care providers, including but not exhaustive of:  specialist doctors, allied health professionals, hospitals and other members of a care team.  In the nature of referrals to specialists, allied health professionals and hospitals, before the referral is made, your consent will be elicited.  In the event of an emergency, all necessary information will be forwarded to paramedics and emergency departments that relates to your specific care and need, or if it relates to public health and safety.

Particular information may have to be passed on to certain public health bodies in relation to notifiable diseases.  You will always be notified if anything has been shared of this nature.

We will not be held liable, if, for any reason, you have withheld or mislead us about any information.  Medical care will always be based on the information we are provided.  E.G. If allergies or medical conditions are withheld from the practicing doctor, there could be serious consequences with contraindications and/or reactions.
